This book, edited by a team of leading European bioethicists, is in all respects an innovative publication. As part of the core materials project of the European Ethics Network, this book collects European perspectives on health care ethics reflecting both the rich philosophical tradition and the broad interdisciplinary network in the field of European health care ethics. In the first part of the book on the physician-patient relationship, the authors present different views on the integration of patient autonomy in the relational structure of the medical profession. Here, the focus is on the reception of patient autonomy in the European context and on European alternatives for the radical understanding of patient autonomy. In the second part of this book, the contributions deal with the ethical problems of clinical research with patients and with the ethical values involved in human experimentation. In the third part of the book, the just organisation of the health care system and the consequent resource allocation are critically examined. With their common European background, the authors want to enrich and encourage the ongoing discussion on the physician-patient relationship, human experimentation and justice in health care from various perspectives and different points of departure. Therefore, this volume is suitable for the interested reader as well as for educational purposes

L'engagement médical implique toute une philosophie. Une métaphysique, parce que la médecine trouve sa raison d'être dans le constat de la réalité des maux qui affligent les vivants. Une épistémologie, parce qu'une connaissance du normal et du pathologie est la nécessaire condition d'une lutte intelligente contre ces maux. Des dilemmes moraux, parce que cette lutte associe la recherche du bien des malades individuels, le respect de leur autonomie, et la prise en compte de l'intérêt collectif. C'est cette philosophie de l'acte médical que les essais ici réunis entreprennent d'expliciter, en abordant notamment les défis méthodologiques et éthiques de cet art, tout armé de technologies et au carrefour de multiples sciences, qu'est la médecine. L'auteur, philosophe et médecin, expose avec rigueur et clarté les stratégies utilisées par la recherche médicale pour détecter, identifier et classer les élément pathogènes (étiologie des affections, logique de l'inférence diagnostique, recherche épidemiologique), les procédures employées pour évaluer les coûts et bénéfices des interventions thérapeutiques (notion de qualité de vie), et les problèmes moraux soulevés par la mise à disposition de services de santé (procréation médicalement assistée, suivi de la grossesse). De cette lecture, on sort convaincu que la sagesse médicale tient à un fragile équilibre entre dévouement à ceux qui souffrent, rationalité incluant l'acceptation du risque, et lucidité sur les limites de nos connaissances.

At first glance, Western humanism, Japanese Buddhism and modern science have so little in common that the very idea of seeking common ground through dialogue seems overly idealistic. Only a man the calibre of Daisaku Ikeda could bring such a project to fruition. "Le médecin doit remplir plusieurs missions : affirmer le diagnostic, fixer les indications thérapeutiques, transmettre la vérité au malade, lui expliquer les raisons de ses décisions. La médecine du passé est, pour une très large part, restée une médecine thérapeutique s'efforçant, selon les préceptes hippocratiques, de retarder la mort et de diminuer la souffrance. La médecine du futur, très différente, sera pour une large part préventive avec, le long de chaque voie, des questions éthiques nouvelles ou renouvelées. Le médecin apprend pendant ses études et tout au long de sa vie, or l'éthique ne s'enseigne pas comme la chimie : des méthodes particulières doivent être proposées et, dès maintenant, un très vigoureux effort d'éducation doit être entrepris. La biologie moderne montre qu'entre les hommes il n'y a pas inégalité mais différence, que tel caractère avantageux ici est désavantageux là (ainsi telle anomalie de l'hémoglobine protège contre le paludisme). Cette éducation apportera aux générations futures les informations nécessaires, tant du côté de la biologie que du côté de l'éthique et leur permettra d'allier les progès de la connaissance à ce respect de la personne qui gouverne toute la bioéthique".
